Primary Care

Medical Services Team

As one of Ontario’s growing Community Health Centres (CHCs), Carea CHC provides comprehensive primary health care that emphasizes accessibility, health promotion, illness prevention and continuity of care for clients with complex physical and mental health needs. Carea CHC is an LGBT-positive space. Specialized care is being offered for trans and gender diverse youth and adults, including referrals for gender-affirming surgeries, administering of gender-affirming hormones, and trans-positive primary care. Other supports for LGBT clients include individual therapy, the Gender Journeys Group program, Colours LGBT Youth Drop In and more.

Our medical services team includes Family Physicians, Nurse Practitioners and Registered Nurses;

Family Physicians:  Our physicians provide comprehensive medical care for patients and their families within our community. Often they are the first people we turn to with our medical problems.

Nurse Practitioners (NPs):  Carea CHC’s NPs are Registered Nurses (RNs)  with advanced university education and experience.  In particular, NPs are able to order and interpret diagnostic tests, communicate diagnoses with our clients, prescribe pharmaceuticals and perform specific procedures.  Our NPs can help with annual physicals, counselling, health promotion (e.g., helping people to quit smoking), immunizations and screening for disease, treatment of short term acute illnesses (e.g., infections) and monitor clients with stable chronic illnesses.

Registered Nurses (RNs):  RNs are focused on the promotion of health, assessment and the provision of care and treatment of health conditions.  They provide supportive, preventive and therapeutic services in order to help our clients attain or maintain optimal health.

Our entire team is dedicated to helping our clients access the best care possible.  We will also help you find other health and social services that you might need.  We work on the wholistic approach of treating the mind.  Just ask us for what you need and we will help you get it.

Administration Support Team

Medical Secretaries and Receptionist:  Carea CHC’s Administration Support Team (AST) are an integral and important part of the Clinical Services team.  They are the first people you see and speak with when you call or visit us.  They always offer a smile.  In a non-judgmental, helpful approach, the AST Team will check you in for your appointments or help you navigate our services and programs.  They provide important assistance to both our Medical Services Team and Counselling Services team booking appointments with specialists, diagnostic tests, managing and maintaining client records and managing the schedules of all the members of our Clinical Services Team just to name a few of their important responsibilities.
